Mechanism of Action

Tadalafil, the active component of tadalafil, is a selective phosphodiesterase type 5 (PDE5) inhibitor and an angiotensin II receptor blocker (ARB). The primary mechanism of action of tadalafil in PAH is its vasodilatory properties. This action results from an inhibition of the phosphodiesterase type-5 (PDE5) enzyme, which is necessary for the degradation of cyclic guanosine monophosphate (cGMP) and is responsible for smooth muscle relaxation in the pulmonary vascular smooth muscle. Tadalafil also enhances the release of prostaglandins (PG) by the corpus cavernosum and vasodilates the corpus cavernosum, resulting in increased blood flow to the pulmonary vessels. In addition, this effect of tadalafil has been demonstrated in patients with PAH and in normal subjects.

Pharmacokinetics
